In order to establish an offshore trust (with exception to purpose trusts) you need to decide exactly who the beneficiaries are, which assets are to be held by the trust, what remit the trustee has and what exactly you want the trust to do.
Usually a written document is drafted called the trust deed, which includes the decisions made above, and is cross examined by a lawyer. The final step is to appoint a trustee and have them sign the document and then sign it yourself.
